Position Overview

Refresh & Motion Physical Therapy is seeking a dedicated and compassionate full-time Physical Therapist to join our dynamic team at our rehabilitation center located in Perth Amboy, New Jersey. In this key role, you will be responsible for evaluating and treating patients with a variety of physical conditions to help them restore function, enhance mobility, and improve their quality of life. Key Responsibilities
  • Conduct comprehensive evaluations of patients, including a review of medical histories and a series of physical assessments to develop an individualized treatment plan.
  • Communicate evaluation findings and treatment protocols in clear language that engages patients and their families actively in the goal-setting process.
  • Implement evidence-based treatment protocols tailored to each patient's unique needs and progress, adapting approaches as necessary to meet changing health conditions.
  • Collaborate with a multidisciplinary team of healthcare professionals to ensure a holistic approach to patient care and timely adjustments to care plans, including consultations as necessary.
  • Oversee discharge planning and follow-up care, ensuring thorough documentation of treatment activities, including daily progress notes and discharge evaluations, to facilitate continuity of care.
  • Utilize effective time management strategies to meet productivity expectations, maintain an accurate schedule, and demonstrate punctuality for team meetings and patient appointments.
  • Contribute to departmental goals by participating in quality improvement initiatives and complying with all relevant governmental and accreditation standards.
  • Take the initiative to pursue continuing education for professional growth, serving as a mentor and clinical instructor for students in the field.
  • Promote an exceptional patient experience by remaining adaptive and responsive to patient needs and preferences, enhancing accessibility to our services.
Patient Population / Client Focus

You will work with a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or dealing with acute injuries. The patient demographic spans various age groups, from pediatric to geriatric populations, allowing for varied clinical experiences. Our focus is on providing personalized care that respects each patient's goals and motivations, ensuring they are at the center of their rehabilitation process.

Qualifications
  • Doctorate degree in Physical Therapy (DPT) is required.
  • A valid DPT license in the State of New Jersey is mandatory.
  • Basic Life Support (BLS) certification for Healthcare Providers from the American Heart Association is required.
  • Active engagement in professional development and continuing education is essential.
Preferred Experience
  • A minimum of two years of relevant physical therapy experience is preferred, though new graduates with a solid educational foundation are encouraged to apply.
  • Demonstrated experience in outpatient therapy settings can be an asset.
