Position Type: Full Time

Haven Home Health & Hospice is seeking a compassionate and organized Clinical Coordinator (LPN) to join our team. The Clinical Coordinator serves as a vital link between patients, caregivers, physicians, and clinical staff to ensure quality patient care and effective communication.

Responsibilities
  • Coordinate patient care and communication between physicians, clinicians, patients, and caregivers
  • Process physician orders and patient care updates
  • Assist with patient admissions and ongoing care coordination
  • Monitor and communicate changes in patient condition
  • Maintain accurate patient records and documentation
  • Support clinicians with scheduling, orders, and care planning
  • Participate in interdisciplinary team communication
  • Assist with orientation and training of new staff
  • Participate in on-call, weekend, and holiday rotations as assigned
Qualifications
  • Current Oklahoma LPN license required
  • Home Health experience preferred
  • Previous clinical coordination experience preferred
  • Strong communication and organizational skills
  • Ability to multitask in a fast-paced environment
  • Proficient computer and documentation skills
  • Valid driver's license, reliable transportation, and current auto insurance
Schedule
  • Full-Time
  • Monday-Friday
  • On-call rotation required
