Progressive Directions, Inc. (PDI) is seeking compassionate, dependable individuals or families interested in becoming Family Model Providers for adults with intellectual and developmental disabilities.
As a Family Model Provider, you will welcome an individual into your home and provide a safe, supportive, family-like environment while helping them achieve greater independence, dignity, and community inclusion. Responsibilities
  • Provide a safe, stable, and welcoming home environment.
  • Assist with daily living skills and personal growth.
  • Support community participation and social activities.
  • Encourage independence, self-determination, and personal choice.
  • Assist with transportation to appointments, activities, and community events.
  • Coordinate with PDI staff, nursing staff, and support teams.
  • Maintain required documentation and participate in training.

Requirements
  • Must be at least 21 years old.
  • Have a valid driver's license and acceptable driving record.
  • Pass all required background checks and drug test.
  • Have a suitable home that meets state licensing requirements.
  • Ability to provide compassionate, person-centered support.
  • Previous experience in caregiving, healthcare, education, social services, foster care, or developmental disabilities is preferred but not required.
