Responsibilities:

• Lead and manage the social services department, overseeing daily operations and ensuring compliance with regulatory requirements and company policies

• Conduct comprehensive assessments to identify residents' social, emotional, and psychological needs, and develop individualized care plans to address those needs

• Collaborate with care teams, healthcare professionals, and family members to ensure a coordinated approach to care and effective communication

• Coordinate and facilitate discharge planning, ensuring a smooth transition from hospital to home or alternative care settings

• Develop and maintain relationships with community organizations, home health agencies, and other healthcare providers to ensure access to necessary resources and services

• Monitored residents' progress and adjust care plans as needed to achieve optimal health, physical function, and independence
