Position Summary

Performance Home Health is seeking an experienced Referral Review Nurse to join our growing team. This is a remote-based position. In this role, you will review incoming home health referrals, ensure all clinical documentation is complete and accurate, identify missing information, and coordinate with multiple departments to support timely patient admissions. The ideal candidate is highly organized, detail-oriented, able to prioritize multiple referrals simultaneously, and thrives in a fast-paced environment where accuracy and efficiency are essential.


Responsibilities

  • Review and process incoming home health referrals in real time.
  • Evaluate referrals for completeness, accuracy, and regulatory compliance.
  • Verify physician orders, supporting clinical documentation, diagnoses, and required disciplines.
  • Review clinical records to determine patient needs and identify missing documentation, needed orders, or required clarifications.
  • Apply ICD-10 coding knowledge to validate appropriate diagnoses when applicable.
  • Ensure referrals meet agency admission requirements before acceptance.
  • Communicate professionally with physician offices, case managers, liaisons, patients, caregivers, and internal staff.
  • Collaborate with Intake, Clinical Leadership, Scheduling, Marketing, and other departments to coordinate timely admissions.
  • Request additional documentation, orders, or clarification as needed.
  • Monitor referral status and ensure timely follow-up on outstanding items.
  • Maintain accurate documentation within the electronic medical record and communication systems.
  • Meet departmental productivity and quality expectations while maintaining exceptional attention to detail.
  • Assist with process improvement initiatives and other duties as assigned.


Required Qualifications

  • Active Texas Registered Nurse (RN) license or Nurse Licensure Compact (NLC) license.
  • Previous Home Health Referral Review or Home Health Intake experience required.
  • Strong understanding of home health regulations and referral requirements.
  • Excellent ability to review clinical documentation and identify skilled nursing and therapy needs.
  • Experience interpreting physician orders and supporting clinical records.
  • Strong organizational skills with exceptional attention to detail.
  • Excellent written and verbal communication skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficient computer skills with the ability to quickly learn new software.
  • Experience using Microsoft Outlook, Microsoft Teams, and Microsoft Office applications.
  • Comfortable working independently in a remote environment while remaining highly responsive to team communication.
  • Knowledge of home health documentation requirements.


Preferred Qualifications

  • ICD-10 coding knowledge or experience.
  • Experience with electronic medical record (EMR) systems, preferably Kantime or similar home health software.
  • Experience coordinating referrals across multiple teams.


Physical Requirements

  • Prolonged periods of sitting and computer use.
  • Ability to work remotely with reliable internet service.
  • Ability to communicate effectively by phone, email, and Microsoft Teams.


Work Environment

This is a remote position providing direct patient care in patients’ homes throughout the assigned coverage area. The RN must be comfortable traveling between patient locations and working independently while maintaining close communication with the interdisciplinary care team.


Salary Compensation

$100,000.00
