• Successfully complete all required orientation and annual training in accordance with Division of Developmental Disabilities (DDD), Medicaid, and organizational requirements, and maintain all required certifications.
• Plan, organize, supervise, and evaluate the daily operations of the Community Access Center (CAC), ensuring the delivery of high-quality, person-centered Day Habilitation services in compliance with DDD, Medicaid, and OTC policies and procedures.
• Provide direct supervision, leadership, coaching, and performance management to assigned staff through regular observation, feedback, performance evaluations, staff meetings, and corrective action when necessary.
• Develop and maintain staffing schedules that ensure appropriate participant-to-staff ratios while effectively managing overtime, substitute coverage, vacancies, and labor costs within approved budget parameters.
• Recruit, interview, recommend for hire, onboard, train, and retain qualified staff in accordance with organizational policies.
• Ensure participants receive meaningful, individualized, community-based and facility-based programming designed to promote independence, community inclusion, personal growth, skill development, and achievement of Individualized Service Plan (ISP) outcomes.
• Monitor participant progress toward identified goals and ensure services are delivered in accordance with each participant's ISP, Behavior Support Plan, and other applicable service plans.
• Coordinate and participate in interdisciplinary team (IDT) meetings and collaborate with Support Coordinators, guardians, family members, nursing staff, behavioral supports, therapists and other providers to ensure quality services.
• Ensure participant documentation, including daily service notes, attendance records, behavior documentation, medical documentation, and electronic records in THERAP, is completed accurately, timely, and in accordance with DDD, Medicaid, and organizational requirements.
• Conduct routine quality assurance reviews and audits of participant records, documentation, medication records, program files, and staff documentation to ensure ongoing regulatory compliance.
• Coordinate participant admissions, transfers, and discharges, including facility tours, pre-admission meetings, review of referral documentation, verification of Medicaid authorization, and completion of all required admission paperwork.
• Maintain regular communication with participants, families, guardians, Support Coordinators, and interdisciplinary team members regarding participant needs, progress, concerns, and services.
• Ensure participant rights, dignity, privacy, informed choice, and confidentiality are protected in accordance with DDD regulations, HIPAA requirements, and organizational policies.
• Oversee medication administration procedures and ensure medication documentation, storage, and administration practices comply with DDD regulations and organizational policies.
• Maintain oversight of program finances, including budget expenditures, purchasing, petty cash, community outing expenses to ensure fiscal responsibility.
• Ensure the Community Access Center facility, equipment, vehicles, and program resources are maintained in a clean, safe, organized, and operational condition. Coordinate repairs and maintenance as necessary.
• Promote a culture of safety by ensuring compliance with emergency procedures, infection prevention practices, OSHA requirements, Danielle's Law, and Stephen Komninos' Law.
• Respond appropriately to participant emergencies, behavioral crises, accidents, injuries, transportation incidents, and medical situations, ensuring required notifications, documentation, and follow-up actions are completed promptly.
• Ensure all required Unusual Incident Reports (UIRs) are completed accurately, submitted within required timeframes, and that appropriate follow-up actions are implemented.
• Monitor program quality through regular observation of programming, participant engagement, staff interactions, community integration activities, and compliance with organizational expectations. Implement corrective actions and quality improvement initiatives as needed.
• Develop, coordinate, and implement innovative recreational, educational, volunteer, and community inclusion opportunities that maximize participant independence and quality of life.
• Ensure transportation services are conducted safely and efficiently. When required, transport participants to and from residences and community activities while adhering to all organizational vehicle safety policies.
• Complete personnel documentation including payroll, personnel action requests (PARs), performance evaluations, disciplinary documentation, attendance and other administrative reports accurately and within established deadlines.
• Maintain effective communication with senior leadership regarding program operations, staffing, participant concerns, regulatory issues, budgetary needs, and quality improvement initiatives.
• Perform other duties as assigned to support the mission and operational needs of the organization.
• Willingly and effectively cooperates with OTC, The Department of Human Services, The Division of Developmental Disabilities (DDD), and other licensing or state agency or local municipalities in any inspections and investigations, upon request.
