Job Purpose:

The Community Health Worker (CHW) is responsible for assisting community members, patients and their family's access and navigate culturally responsive community services and resources while also promoting healthy behaviors.

General Job Statement:

The CHW MNsure Navigator position is responsible for providing MNsure enrollment and assistance to families residing in North Minneapolis and surrounding communities. The CHW will raise awareness of the importance of health insurance, accessing preventive healthcare such as medical and dental check-ups, and the social and economic issues impacting families. Outreach activities will be conducted in neighborhoods, agencies, churches, park and recreation facilities, and schools.

Minimum Qualifications:

  • High School Diploma/GED with four (4) years of experience in human services, community outreach, health education, or a related field OR Associate's degree with two (2) years of experience in human services, community outreach, or a related field, required.
  • CHW certification from accredited program, preferred. If not CHW certified, must complete within 2 years from the date of hire.
  • Ability to successfully pass MNsure Navigator background requirements and maintain certification in accordance with MNsure policies and annual training requirements.
  • Bilingual in English and preferably Spanish
  • Demonstrated ability to establish rapport and effectively engage individuals and families from diverse cultural, ethnic, linguistic, and socioeconomic backgrounds.
  • Minimum of one (1) year of experience assisting individuals with healthcare access, public benefits, health insurance enrollment, or community resource navigation, preferred.
  • Experience developing outreach strategies, conducting community education, collecting data, and preparing reports.
  • Knowledge of community resources, healthcare systems, and social service programs serving underserved populations.
  • Excellent verbal, written, analytical, and group communication skills working with a multi-cultural environment.
  • Valid Minnesota driver's license with good driving record active auto insurance.
  • Proficiency in Microsoft Office (Word, Excel, Outlook) and experience with electronic health records and/or client tracking databases (Apricot preferred).
