Position Summary:
Treatment Coordinators with National Youth Advocate Program work in the community as part of an integral service team.ย Under the general supervision of the Clinical Supervisor or designee, this position works closely with youth, family, foster parents, and community partners, providing advocacy and support toward the ultimate goal of living safely and successfully in the community.ย

Responsibilities
โข Provides appropriate individual, group, and family mental health services, and other evidence basedย clinical servicesย as required.ย ย
โข Travels daily, to provide community basedย services to, and on behalf of, youth andย families inย compliance with organizational, contract, and regulatory requirements.ย ย
โข Coordinates and monitorsย clinical servicesย for the youth and family in the community, including mental health,ย specialย educational services, medication management, andย psychological services.ย ย
โข Regularly reviews and modifies youth-basedย plans.ย ย
โข Establishes andย maintains strongย relationship with foster parents andย families of origin.ย ย
โขย Assists in family reunification and stabilizationย efforts with the youthโs primary family or other permanent living arrangement consistent with the youthโs case plan.ย ย
โขย Provides safetyย assessment,ย crisis response, and behavioral stabilization servicesย as required.ย ย
Minimum Qualificationsย ย
โขA Bachelorโs Degree in Social Work from an accredited institution.ย
โข A willingness to work flexible and non-traditional hours as needed.ย
โข Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.ย
Driving and Vehicle Requirementsย
- Valid driverโs license
- Reliable personal transportation
- Good driving record
- Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
