Job description

Company Description

A mission-driven organization, Lutheran Metropolitan Ministry (LMM) serves people in Northeast Ohio and walks with them on their road to self-sufficiency.ย  LMM helps empower people to overcome barriers, obtain job skills, gain employment, locate safe and stable housing, access counseling and support services, stay out of prison, and secure second chances in community.ย  Our mission is to promote shalom (peace, well-being) and justice (right relationships) through a Christian ministry of service and advocacy with those who are oppressed, forgotten and hurting.

Position Summary

Under direct supervision of the Youth and Young Adult (YYA) Navigation Coordinator and general supervision of the VP, YRS; working collaboratively with other Youth Homelessness Demonstration Program agencies, the Navigation Specialist provides individualized, trauma-informed assistance to young adults (16-24) experiencing housing instability or homelessness. The YYA collaborates with program agencies to meet immediate needs and help secure safe housing, prioritizing alternatives to shelter. For individuals experiencing homelessness, they rapidly navigate pathways to permanent housing.

Some of the duties include:

  • Engage and enroll youth and young adults experiencing homelessness directly or through Coordinated Intake, outreach, referral, or By Name List identification
  • Prioritize diverting individuals from shelters, focusing on crisis and emergency situations
  • Uphold the integrity and self-determination of those served by promoting empowerment, addressing needs, and treating everyone with dignity and respect
  • Facilitate intensive case management to prevent homelessness, promote rapid housing placement, and ensure self-sufficiency
  • Educate participants about available services and resources, troubleshooting barriers constructively to ensure engagement
  • Provide crisis intervention services as needed and collaborate on assessments and housing preferences
  • Assist with housing placement, including transportation, landlord engagement, inspections, lease signing, and move-in support
  • Maintain accurate documentation to comply with standards and requirements
  • Actively participate in team meetings and collaborate with partners to expedite housing
  • Adhere to best practices including Trauma-Informed Care, Motivational Interviewing, Housing First, Harm Reduction, and Antiracism

Qualifications:

High school diploma or equivalent, with training in mental illness, psychiatric symptoms, and therapeutic engagement. A Bachelorโ€™s degree in social, behavioral, or human services, with at least one year of supervised work in mental health, chemical dependency, or social services delivery, is preferred. Lived experience of homelessness and/or housing instability, advocating for diverse identities of young adults served, including BIPOC, LGBTQIA+, and Young Adults a plus. Additionally, possessing a valid Ohio Driverโ€™s License with proof of insurance is a requirement for this position.

LMM offers a number of benefits for staff including, but not limited to: medical, dental, vision, disability, life, generous paid time off and a 403(b) retirement savings plan.ย ย  LMM is an Equal Opportunity Employer that values diversity in life experiences and backgrounds. LMM promotes equality for individuals of all backgrounds, including Minorities, Females, Disabled Individuals, and Veterans. LMM also maintains a drug-free workplace.
