Position Title: Functional Wellness Nurse Practitioner or Physician Assistant
Reports To: Medical Director, Functional Wellness Program
Position Summary
The Functional Wellness Provider is responsible for delivering comprehensive preventive and wellness-focused care through a root-cause medicine approach. The provider will evaluate lifestyle, nutrition, metabolic health, hormone balance, and chronic disease risk factors while developing personalized treatment plans designed to optimize health outcomes and improve quality of life.
Essential Responsibilities
- Conduct comprehensive wellness evaluations and annual assessments
- Review laboratory and diagnostic testing related to metabolic, hormonal, cardiovascular, and nutritional health
- Develop individualized wellness plans
- Provide counseling on:
- Nutrition and weight management
- Exercise and physical activity
- Stress management
- Sleep optimization
- Smoking cessation and lifestyle modification
- Manage hormone optimization programs under physician oversight
- Prescribe medications and supplements as permitted by state regulations
- Monitor treatment response and adjust care plans accordingly
- Coordinate care with PCPs, specialists, health coaches, and ancillary providers
- Maintain accurate documentation within EMR
- Participate in quality improvement initiatives and wellness program development
Qualifications
- Licensed Nurse Practitioner or Physician Assistant in Florida
- DEA License
- Minimum 2 years of primary care, preventive medicine, endocrinology, or wellness experience preferred
- Experience with weight management, hormone therapy, and metabolic health preferred
- Strong patient education and motivational interviewing skills
