Role Overview

We are hiring a driven field professional with direct knowledge of Enhanced Care Management (ECM) to grow Tangelo’s partnerships and referral volume across Southern California (with occasional Northern California support).

You will spend the majority of your time in the field meeting ECM providers, Community Supports teams, care managers, and health plan contacts in Los Angeles. This is a high-activity, relationship-driven role focused on opening doors, educating partners, and driving consistent referrals into Tangelo’s meal programs.

Key Responsibilities

Partnership Development

• Identify, engage, and convert ECM providers and Community Supports teams into active Tangelo partners.
• Conduct frequent in-person meetings, presentations, and drop-in visits across Los Angeles (plus occasional Contra Costa visits).
• Build and maintain long-term relationships with ECM lead care managers, Community Supports coordinators, and key decision-makers.
• Develop account plans and expand referral volume within existing partner organizations.

Growth & Pipeline

• Proactively prospect new ECM and Community Supports partnerships through cold outreach, networking, and territory mapping.
• Consistently meet or exceed referral and partnership activation targets.
• Use CRM tools to track activity, pipeline, and account health.
• Identify expansion opportunities (new populations, additional counties, improved referral workflows).

Territory Management

• Own the primary Los Angeles territory and travel daily as needed.
• Support occasional Contra Costa (NorCal) partner visits as required.
• Plan efficient travel schedules and manage T&E within company guidelines.
• Monitor portfolio performance and take action to improve referral rates and partner satisfaction.

Internal Collaboration

• Work closely with Tangelo’s enrollment, member services, clinical, and operations teams to resolve partner issues and ensure smooth member onboarding.
• Provide field feedback to improve processes, materials, and partner experience.

Required Qualifications

• Proven ability to build relationships and drive results through face-to-face engagement.
• Strong communication, presentation, and problem-solving skills.
• Experience with CRM tools (HubSpot, Salesforce, or similar).
• Valid driver’s license and willingness to travel daily across Los Angeles plus occasional trips to Contra Costa.
• High initiative and ability to work independently while collaborating with internal teams.

Preferred Qualifications

• 2+ years of field sales, account management, or partnership development experience in healthcare — preferably with Enhanced Care Management (ECM), Community Supports, CalAIM, Medi-Cal, or managed care.
• Direct working knowledge of ECM programs and how referral pathways operate between ECM providers and Community Supports vendors.
• Existing relationships with ECM providers or Community Supports teams in Southern California (and ideally Northern California).
• Experience with food-as-medicine, medically tailored meals, or nutrition-related benefits.
• Bilingual (Spanish) a strong plus.

• Steady growth in ECM and Community Supports referrals from the assigned territory.
• Strong, trusted relationships with key partners across Los Angeles and occasional Contra Costa support.
• Consistent achievement of referral and activation targets.

Compensation

$60,000 – $70,000 base salary + bonus opportunity
