Job description

Registered Nurse (RN) Case Manager

The Registered Nurse (RN) Case Manager is responsible for overseeing and coordinating healthcare services for individuals with intellectual and developmental disabilities (IDD) in community-based and ICF settings. This role ensures compliance with Washington, DC Department on Disability Services (DDS), Department of Health (DOH), and applicable DDA standards, while promoting person-centered, high-quality care.

Key Responsibilities

  • Conduct comprehensive health assessments and develop individualized nursing care plans
  • Participate in Individual Support Plan (ISP) and Interdisciplinary Team (IDT) meetings
  • Ensure compliance with DDS, DOH, DDA, and ICF regulatory requirements
  • Review and monitor Medication Administration Records (MARs) and physician orders
  • Provide clinical oversight and delegation to DSPs and LPNs per DC regulations
  • Train staff on health supports, medication administration, and documentation standards
  • Monitor incident reports and follow up on reportable health events per DDS guidelines
  • Maintain accurate documentation aligned with audit and compliance standards

Requirements

Regulatory Compliance & Quality Assurance

Ensure compliance with:
  • DDS and DOH regulations
  • Federal ICF/IDD standards
  • OSHA guidelines
  • Assist in the development of clinical policies, protocols, and quality improvement initiatives
Collaboration & Communication
  • Communicate consistently with interdisciplinary teams to ensure continuity of care
  • Attend all required nursing and clinical meetings
  • Support coordination across residential, day, and clinical services

Minimum Qualifications

  • Active Registered Nurse (RN) License in Washington, DC
  • Minimum 2 years of nursing experience
  • Strong knowledge of nursing practices and care coordination
Additional Responsibilities
  • Ensure availability of medical supplies and resources
  • Maintain current licensure documentation for all nursing personnel
  • Perform other duties as assigned

Preferred Qualifications

  • Experience in IDD, ICF, or community-based services
  • Knowledge of DDS, DDA, DOH, ISP, HCMP, and IDT processes
  • Familiarity with DDS, DOH, or DDA regulations
  • Experience supervising or training staff
  • Experience with EHR systems

Work Environment

  • Field-based role across multiple residential and program locations, with administrative work
  • Local travel required (mileage reimbursement provided)
  • Monday–Friday schedule with On-call rotation required
