Job description

Job Title: Case Manager

Program/Department: Malden TIP

Position Type: Non-Exempt, Full-Time 40 hours/week – Hybrid Position (remote and on-site)

Location: Salemwood K-8 School in Malden 

Reports to: Clinical Supervisor

IHR’s CARF accredited treatment programs provide holistic interventions for hard to reach and high-risk individuals and families. IHR’s mission, since our founding in 1990, has been to develop and support a comprehensive continuum of care for families, individuals, youth, and pregnant and parenting people affected by alcohol, tobacco, and other drug use, violence/trauma, mental health challenges and other health issues, while advancing principles of health equity and social justice. Our work is focused on addressing and eradicating inequities in our health care, economic, and justice systems, especially as they disproportionately affect women and people of color. We are committed to providing services and creating communities that mitigate the effects of substance use, mental illness, and trauma. 

 

Summary Statement: The Case Manager will support Malden TIP (Teach Inspire Prepare) Program, an innovative elementary and middle school-based program (grades 4-8), and IHR’s mission to support youth and families. The Case Manager will work closely with the Case Management team, including clinical supervisor, clinical case manager, and school personnel, to deliver targeted prevention and intervention programs to students demonstrating concerning behaviors in school and the community.


Essential Functions:
  • Provide clinical case management services to youth enrolled in Malden’s Salemwood School.
  • Outreach to enrolled youth and their families including Salemwood personnel, caregivers, state agencies, and additional referral sources to provide program information
  • Conduct intakes and needs assessments for program participants.
  • Provide case management services to 15-20 youth and their families per year using strengths-based and trauma-informed approaches.
  • Engage in care coordination with families, school personnel, community providers, and other collaterals.
  • Develop, implement, and review Individual Service Plans and review based on program requirements.
  • Proactively identify and make referrals to existing community resources to support various life domains.
  • Proactively plan and facilitate afterschool and summer activities for participants and their families.
  • Provide individual mentoring to participants and lead developmentally appropriate activities. 
  • Develop, maintain, and update lists of community resources to ensure timely and appropriate referrals.
  • Co-facilitate psychoeducation groups that promote life skills training and strategic peer involvement.
  • Support family service efforts through family education and family nights.
  • Maintain and adapt schedule based on needs of programming, including after business hour availability to facilitate youth and family activities. 
  • Actively work with school personnel to provide education advocacy, tutoring and homework support.
  • Utilize incentives to engage clients in program activities.
  • Regularly collaborate with Malden TIP staff, and other youth direct care team staff, to support overall requirements of grant and youth team missions. 
  • Maintain all written reports and records (including electronic health records) accurately and in a timely manner.
  • Meet regularly with Supervisor for clinical and administrative supervision.
  • Use cloud-based systems for communication with IHR’s site office, including shared calendars, timesheets, and supervisor check-ins. 
  • Attend youth team meetings, agency staff meetings, and grant related meetings, as appropriate. 


Qualifications/Requirements:
  • Bachelor’s degree required.
  • Previous experience working with adolescents/transition-age youth, families, mental health, and substance use issues.
  • Previous experience providing treatment at home, school, or in community settings.
  • Strong familiarity with assigned catchment area communities and local resources.
  • Competency in working with diverse individuals, families, and cultural backgrounds.
  • Demonstrated oral and written communication skills.
  • Energetic, flexible, and able to take initiative and work independently as well as part of a team.
  • Must have regular access to a reliable vehicle and a valid Massachusetts Drivers’ License.
  • Bilingual/Bicultural (Spanish/English) candidates are strongly encouraged to apply.
