Position Summary

Hope’s Destiny is seeking a licensed therapist to join our multidisciplinary team. The ideal candidate has experience providing outpatient therapy to adolescents and/or adults with substance use disorders and co-occurring mental health conditions and demonstrates a strong commitment to delivering high-quality, client-centered care.

Responsibilities
  • Conduct comprehensive biopsychosocial assessments to evaluate clients’ mental health, substance use, and treatment needs.

  • Develop and implement individualized treatment plans based on assessment findings, client goals, and evidence-based practices.

  • Provide evidence-based therapy, including individual, group, and family sessions, utilizing modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Motivational Interviewing (MI), and trauma-informed approaches.

  • Facilitate psychoeducational groups on substance use, mental health, relapse prevention, and coping skills.

  • Collaborate with the multidisciplinary team, including psychiatric providers and case managers, to ensure integrated care.

  • Conduct crisis intervention and risk assessments, providing appropriate referrals and resources when needed.

  • Maintain accurate, timely clinical documentation in compliance with state regulations and agency policies.

  • Participate in team meetings, case conferences, and clinical supervision.

  • Stay current with best practices through continuing education and professional development.

Required Education & Licensure
  • Master’s degree or higher in counseling, social work, psychology, or a related behavioral health field.

  • Current, active licensure in the state of Pennsylvania as one of the following:

    • Licensed Clinical Social Worker (LCSW)

    • Licensed Professional Counselor (LPC)

    • Licensed Marriage and Family Therapist (LMFT)

    • Or equivalent independent clinical license

Qualifications
  • Minimum of 2 years of experience providing therapy for substance use disorders and co-occurring mental health conditions in an outpatient setting.

  • Strong knowledge of evidence-based and trauma-informed treatment approaches.

  • Demonstrated expertise in assessment, diagnosis, treatment planning, and clinical intervention.

  • Ability to build rapport and engage clients from diverse backgrounds.

  • Proficiency with electronic health records (EHR) and clinical documentation standards.

  • Experience working with higher levels of care (IOP, PHP) preferred.

Working Conditions
  • Primarily office-based clinical environment.

  • Occasional travel for meetings, trainings, or conferences may be required.

  • Flexible scheduling, including evening or weekend sessions, may be necessary.

  • Must maintain confidentiality and professionalism in a fast-paced, client-centered setting.

Physical Demands
  • Occasional standing for extended periods.

  • Ability to lift and/or move up to 25 pounds as needed.
