Job summary
We are hiring a Licensed Practical Nurse (LPN) for a 13-week evening shift assignment at Hillcrest Center. The LPN will provide direct patient care, support individualized care plans, and collaborate with the clinical team to ensure safe, high-quality patient outcomes.

Assignment details
• Shift: Evening (Shift 2)
• Schedule: 3:00 PM – 11:30 PM
• Meal Break: 30 minutes
• Setting/Area: Hillcrest Center
• Order Reason: New Opening
• Job Type: Contract/Temporary

Pay rate
• $35.53/hr

Key responsibilities
• Provide direct nursing care within LPN scope under RN/physician direction
• Administer medications and treatments per orders and facility policy
• Perform wound care, dressing changes, catheter care, and specimen collection as assigned
• Monitor patient condition and promptly report changes to the RN/clinical supervisor
• Document care accurately and on time in the facility system
• Follow infection control, safety procedures, and HIPAA requirements
• Communicate effectively with patients, families, and interdisciplinary staff

Required qualifications
• Active LPN license (in good standing)
• 1+ year recent LPN experience (SNF/LTC/rehab/hospital preferred)
• Current BLS/CPR (AHA preferred)
• Strong communication and documentation skills
• Reliable attendance for the full evening schedule

Compliance requirements (as applicable)
• Background check and drug screen
Immunizations/health screenings per facility policy
• Ability to complete onboarding and credentialing within required timelines
