Target Cash Compensation Band: $165,000$245,000 (Cash, plus bonus, ownership stake, and equity opportunities)
The OpportunityWe're seeking a Medical Director (MD/DO) to serve as the senior-most physician at Haven Health's Los Angeles practice. This is a dual role: you'll manage your own patient panel, delivering hands-on primary care, and you'll hold clinical and regulatory oversight of the practice, including supervision of physician assistants and nurse practitioners, ownership of clinical protocols, and quality assurance. You'll partner directly with the CEO and practice leadership on clinical strategy, and you'll have the opportunity to grow into a stake in practice ownership over time. This is a unique opportunity to combine patient care with clinical leadership at a mission-driven startup, with a real path to ownership and other equity ownership opportunities.
What You'll DoClinical Care
- Provide comprehensive primary care, including acute, chronic, and preventive visits.
- Deliver and oversee Haven's clinical service lines, including semaglutide/GLP-1 and B12 therapy, sermorelin (injectable and sublingual), enclomiphene, oral testosterone and other hormone therapy, NAD+ injectable, glutathione sublingual, and alprostadil/trimix for erectile dysfunction.
- Diagnose, treat, and manage a wide range of conditions (e.g., hypertension, diabetes, STIs, mental health, gender-affirming care).
- Perform in-office procedures when appropriate.
- Order and interpret labs, diagnostics, and imaging; manage prescriptions; and complete documentation per Haven Health clinical protocols.
Telehealth & Virtual Care
- Deliver patient care through secure telehealth visits and asynchronous communication.
- Manage patient follow-ups, clinical inboxes, and continuity of care across digital and in-person settings.
Medical Director & Clinical Oversight
- Serve as the practice's senior physician leader, setting clinical protocols and standards of care in partnership with the CEO.
- Provide the required level of clinical supervision for all Practitioners at the Clinic in accordance with California law, including collaborative/supervisory practice agreements with physician assistants and nurse practitioners (subject to California's supervision limits per physician).
- Establish and oversee the practice's quality assurance program, including chart review, utilization review, and quality improvement initiatives.
- Maintain all required licensure, board certification, DEA registration, and reporting obligations, and promptly report any change in status per applicable regulations.
- Mentor and collaborate with physician assistants, nurse practitioners, medical assistants, and other care team members; champion a culture of teamwork, inclusion, and affirming patient service.
Quality, Innovation & Growth
- Track and contribute to clinical quality and performance goals.
- Support the rollout of new treatments and services (e.g., allergy therapy, wellness offerings, preventive pilots).
- Participate in research, innovation initiatives, and care model testing.
- Help refine and expand Haven Health's playbooks and SOPs for national replication.
Ownership & EquityThis role comes with a genuine path to ownership, structured in two parts:
- Practice (PC) Ownership Track: As California law requires that a professional medical corporation be owned by a licensed physician, this role includes the opportunity to become a physician-owner of Haven Health Group Los Angeles, P.C. over time, subject to applicable corporate practice of medicine requirements and definitive documentation.
- Service Organization Equity: Separately, this role is eligible for equity participation in Haven Health's management services organization (the entity that supports the practice's growth, technology, and operations), giving you a stake in the broader company's success beyond the practice itself.
Specific vesting terms, buy-in mechanics, and timelines will be discussed directly with qualified candidates as part of the offer process.
Who You AreMust-Haves (Non-Negotiables)
- MD or DO with a current license in California.
- Board-certified in Family Medicine or Internal Medicine.
- 3+ years of experience in primary care, family medicine, or related outpatient specialty (new graduates with residency training in primary care considered).
- Demonstrated experience or strong interest in serving LGBTQ+ patients.
- Proficiency with EHR systems and telehealth workflows.
- Excellent diagnostic and clinical judgment, especially in an outpatient setting.
- Strong interpersonal skillsable to build trust across diverse patient populations.
- Commitment to confidentiality (HIPAA), safe prescribing, and regulatory compliance.
- Active DEA registration in good standing.
- Willingness and ability to serve as a supervising physician for physician assistants and/or nurse practitioners under California law.
