**Responsibilities**

* Provide direct patient care to residents in a rehabilitation setting, including assessing, planning, implementing, and evaluating individualized care plans
* Collaborate with healthcare teams to develop and implement effective treatment plans to achieve optimal patient outcomes
* Administer medications, treatments, and procedures as prescribed by physicians and in accordance with established protocols
* Monitor and record patient vital signs and medical records, reporting any changes or concerns to the healthcare team
* Educate patients and their families on disease processes, treatment options, and self-care techniques
* Participate in quality improvement initiatives to promote evidence-based practice and patient safety

**Requirements**

* Current Registered Nurse (RN) license in the state of Indiana
* BSN degree in Nursing or related field
