Job description

Job Title

To provide stabilization of community-based adult, children, adolescence and discharged patients after experiencing a crisis episode. Responsible for the provision of community transition services, works collaboratively with case manager to ensure the integration of services and client assessment needs that lead to specific treatment recommendations.

Qualifications

  • Graduate degree in behavioral health field
  • Minimum of two (2) years' experience in professional clinical counseling and or therapy
  • Have valid LCSW, LCPC, or LMFT license(s)
  • Must be IM+ CANS certified
  • Possess exceptional written and verbal communication skills
  • Proficient in Microsoft Word and Excel software

Responsibilities

  • Provide comprehensive mental health assessment, treatment planning, therapeutic services, case management and treatment evaluation for person served.
  • Provide trauma informed brief therapy for individuals to ensure stabilization and transitions care from hospital.
  • Completes IM+CANS, treatment plans, progress notes and other mental health assessment documentation.
  • Provide linkage, care coordination, and resource services.
  • Directs transition planning and aftercare follow up for person served.
  • Work collaboratively with outpatient clinical and community service providers as well as Habilitative Systems, Inc teams.
