    Job description

    Community Living Support Services (CLS) are designed to assist individuals with intellectual developmental
    disabilities with developing, preserving, or improving the skills required to live as independently as they
    possibly can while ensuring continuity, safety and high-quality service in the individual's personal
    residence or family home.
    InCommunity provides Community Living Support services in compliance with the Department of
    Community Health (DCH) and DBHDD waiver programs. CLS services may include personal
    care/assistance. The amount of personal care/assistance is specific to the needs of the Individual, as
    determined by the Supports Intensity Scale, the Health Risk Screening Tool, and other person-centered
    assessments. The service needs may be addressed in one or more of the following areas: Activities of
    daily living, personal grooming and health care, interpersonal relationships, mobility, home management,
    and use of leisure time.
    Essential Duties & Responsibilities
    -Community Access & Person-Centered Services
    -Provide training and support to the individuals in all daily living skills including but not limited to
    nutrition, personal hygiene, health care, safety, communication, interpersonal relations, mobility,
    financial management, home management and use of leisure time as outlined in the individual
    service plan (ISP).
    -Ensure individuals are taking medication according to prescribed schedule.
    -Schedule medical appointments as directed by the Program Manager or Family.
    -Accompany individual and facilitate participation in visits for medical care, therapies, personal
    shopping, recreation and other community activities as needed.
    -Provide training and support in the areas of social, emotional, physical and personal skill
    development.
    -Assist in the development of ISPs as requested by the Program Manager.
    -Provide transportation in personal vehicle for individuals as directed by the Program Manager.
    -Communicate regularly with Program Manager on individual's progress, issues and concerns.
    -Other duties may be assigned.
    Service Planning & Documentation
    -Provide respectful, person-centered services and support to individuals in alignment with their Individual Service Plans (ISP).
    -Maintain a relationship with and communicate with families and key supports regarding the individual's day-to-day needs and successes.
    -Collaborate with individuals, families, and interdisciplinary teams to assist in the development of the
    ISP.
    -Documentation of the individual activities addressing goals and progress toward ISP goals.
    -Maintain and submit all required paperwork in a timely manner in Therap online portal.
    -Documentation and other tracking required by the ISP, including Behavioral Support Plan, Health
    Care Plan or medical needs.
    -Collaboration with the Program Manager and InCommunity Team members for any significant
    changes or updates to ensure the health, safety, and welfare of the individual supported.
    Advocacy & Support
    -Empower individuals with IDD to advocate for themselves in the community settings.
    -Provide guidance and support to individuals and staff in addressing barriers to independent
    community living.
    -Collaborate with team members, families, and community partners to enhance outcomes and
    develop natural supports in the individual's community.
    -Promote a culture of inclusion, dignity, and respect in all service areas.
    Health and Safety & Compliance
    -Reporting all changes or concerns to the Program Manager within one hour of observation.
    -Support safety and well-being through adherence to health, safety, and emergency protocols.
    -Assure confidentiality and individual rights requirements are maintained and aligned with the federal,
    state, DBHDD, and agency guidelines.
    -Assure successful completion of all initial and annual training requirements required for Community
    Living Support services as outlined by the DCH and DBHDD.
    -Participate in staff training, meetings, and professional development.
    -High school diploma or equivalent (additional relevant training preferred).
    -2+ years of experience working with individuals with intellectual/developmental disabilities.
    InCommunity
    -Strong communication and interpersonal skills.
    -Proficiency with basic computer use and documentation.
    -Personal Vehicle and valid Georgia driver's license.
    -CPR/AED and First Aid certification (or ability to obtain within 90 days).
    -Must be 21+, able to lift up to 50 lbs., pass a pre-employment physical, TB screening, drug
    screening, MVR, and background check.
