Known as Four Corners, this spot is the site of a Navajo Tribal Park. Pay your admission, and you, too, can experience the Four Corners state of mind. However, Four Corners is much more than a surveyor's gimmick. The term also refers to this entire region, most of which is Navajo and Hopi reservation land. The Four Corners region happens to have some of the most spectacular landscapes in the state, with majestic mesas, rainbow-hued deserts, towering buttes, multicolored cliffs, deep canyons, a huge cliff-rimmed reservoir, and even a meteorite crater. Among the most dramatic landscape features are the 1,000-foot buttes of Monument Valley, which for years have symbolized the Wild West of John Wayne movies and car commercials.

The Facility – Four Corners Health Center
The Four Corners Regional Health Center (FCRHC) is located in Red Mesa, Arizona near the borders of Utah, Colorado and New Mexico. FCRHC opened in January 2007 and is under the administrative oversight of Shiprock Service Unit. FCRHC provides ambulatory care, sub-acute care and community health services in the following areas: family medicine, internal medicine, optometry, dental, pharmacy, radiology, physical therapy, laboratory, social services, case management, mental health, public health nursing; diabetes education, health promotion and disease prevention; and traditional medicine.
Requirements (Duties include but are not limited to)
  • Assesses patients and develop plan of care for patients who present to the clinic. Triage patient and evaluate chief complaint. Initiate treatment measures based on appropriate utilization of standing orders, certifications and standards of care for the clinic.
  • Consult with attending provider and collaborate in developing multi-disciplinary approach to provision of care. Coordinate available resources to provide patient and family with social, age specific, emotional, educational, cultural, spiritual and other identified health care support system needs.
  • Observe and assess patient clinical condition- collect and interpret data from the history and physical assessment. Based on interpretation of data, implements appropriate Nursing action and informs the provider of the patient's status.
  • Assists charge nurse in maintaining patient flow and efficiency in order to ensure safety, patient confidentiality and to expedite onset of treatment.
  • Administers therapeutic measures as prescribed by the provider to include but is not all-inclusive to medication administration, IV fluids, and other medical treatment to provide and meet the patient health care needs.
  • Documents all relevant patient health care treatments according to the organization's policies and procedures and in compliance with the organization and accrediting bodies health care standards of care.
  • Maintain professional knowledge and proficiency in unit specific nursing knowledge and practice through continuing education. Participate in development updating and evaluation of standards of care and quality improvement measures of the organization. Attends and participates in department staff meetings and in-services to comply with ongoing training and educational requirements for the accrediting agencies and the organization.
  • Maintain a professional caring attitude when dealing with patients, their families , hospital health care providers and other departments.
  • Works on an assigned or rotational basis to provide continuity of services during the day and will help cover, if assigned, to stay after hours to assist in completing patient visit with the providers.
 
Qualifications
  • Candidates must possess a bachelor's degree in Nursing and eligibility under the Superior Academic Achievement provision -OR- Possess of a bachelor's degree in Nursing and at least one full year of professional nursing experience-OR- Possess a diploma or associate degree in professional nursing
  • Must possess a current, valid, unrestricted nursing license in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.
  • Must have a minimum of 12 months experience in a Primary Care environment during the last 36 months
  • Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) as approved by the American Heart Association (AHA) is mandatory for all specialties.
  • Evidence of current immunizations is required
