Work Environment

This position is primarily onsite, providing home care services within the community. You will work closely with clients in their own environments, which allows for personalized care tailored to their specific needs and circumstances. Our work schedule runs from Monday to Friday, 8 AM to 3 PM, allowing for a balanced professional and personal life.

Key Responsibilities
  • Conduct comprehensive evaluations of patients to assess physical condition and limitations.
  • Develop individualized treatment plans based on evidence-based practices and patient-specific goals.
  • Implement therapeutic interventions, utilizing modalities such as exercises, manual therapy, and adaptive techniques to promote recovery.
  • Regularly reassess clients to evaluate effectiveness of treatment and make necessary adjustments to therapy plans.
  • Document all patient interactions and progress thoroughly in the Company's electronic record system, including SOAP notes and care plans.
  • Educate patients and their families on the recovery process, addressing challenges and providing strategies for improvement.
  • Meet departmental productivity goals and support the overall business objectives of Flexibility Plus Rehab.
  • Collaborate effectively with interdisciplinary teams to ensure comprehensive care for clients receiving multiple services.
  • Promote and embody the core values of Flexibility Plus Rehab while acting as an ambassador for our organization.
Patient Population / Client Focus

Qualifications
  • A Master's degree or Doctor of Physical Therapy (DPT) from an accredited physical therapy program is required.
  • Current and unrestricted licensure from the Board of Physical Therapy in Connecticut.
  • Willingness to provide both in-person and telehealth evaluations and treatments as clinically appropriate.
  • A passion for working with diverse populations and a commitment to improving patient outcomes.
  • Adaptability and flexibility in a dynamic work environment, emphasizing a collaborative approach.
  • Proficient in using electronic health record (EHR) systems for documentation and management of patient care.
Preferred Experience
  • Experience in pediatric and geriatric physical therapy is advantageous.

  • Proven track record of collaboration within a healthcare team to optimize patient care.
Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience and qualifications.
