Key Responsibilities
  • Conduct comprehensive evaluations to assess patient needs, including reviewing patient history and performing specific assessments to create customized treatment plans.
  • Clearly explain evaluation findings and treatment plans to both patients and their families, actively involving them in the goal-setting process.
  • Implement treatment protocols effectively based on the initial evaluations and the evolving needs of each patient, adapting as necessary in response to their progress.
  • Collaborate with fellow healthcare professionals to ensure a holistic approach to patient care, modifying treatment plans when changes in a patient's condition are noted.
  • Manage discharge planning with attention to follow-up care, documenting all patient interactions thoroughly and promptly to maintain accurate, up-to-date records.
  • Exemplify excellent time management skills while meeting productivity goals, ensuring timely attendance at meetings, and participating in community outreach initiatives.
  • Contribute to departmental goals by adhering to regulatory standards and playing an active role in departmental development and strategy.
  • Engage in ongoing professional development, pursuing continuing education opportunities and mentoring students in clinical placements.
  • Enhance the patient experience by promoting accessibility to care through flexibility and adaptive service delivery.
Patient Population / Client Focus

The Physical Therapist role at Evergreen Physical Therapy Services primarily focuses on outpatient care, catering to a varied patient population recovering from surgeries, injuries, chronic pain, and neurological disorders. Our therapists are equipped to create individualized treatment programs that not only address physical rehabilitation but also encourage patients' emotional well-being and resilience.

Qualifications
  • A Bachelor's Degree in Physical Therapy is required; Doctorate in Physical Therapy (DPT) is preferred.
  • Current state licensure as required for practicing in New Jersey.
  • American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers is mandatory.
  • Although two years of experience is preferred, we encourage new graduates to apply.
Preferred Experience

While a minimum of two years of experience is desired, we are open to considering new graduates who are eager to learn and grow within our team. Experience in outpatient settings or familiarity with therapeutic modalities is advantageous but not required.
