Job Description:

We are seeking a highly organized and clinically skilled Nurse Triage professional to join our team. The ideal candidate will provide patient-centered telephonic and electronic nursing support in a fast-paced, multidisciplinary healthcare environment. This role requires strong clinical judgment, exceptional communication skills, and the ability to independently triage patient concerns using established protocols and provider guidance.

Key Responsibilities:

  • Independently assess and triage patient symptoms and concerns utilizing approved clinical protocols, nursing judgment, and provider-directed care pathways.
  • Manage high-volume live calls through a multi-line call system while maintaining professionalism, accuracy, and efficiency.
  • Monitor, retrieve, and return patient voicemails within established policy-directed turnaround times.
  • Navigate multiple electronic systems simultaneously, including EMR, patient communication platforms, and live call systems.
  • Utilize EMR for documentation, prescription refill processing, appointment scheduling, care coordination, and patient communication.
  • Respond promptly and professionally to patient inquiries received through the patient portal.
  • Coordinate care with physicians, advanced practice providers, residents, fellows, ancillary departments, pharmacies, and external healthcare organizations to support continuity of care.
  • Process prescription refill requests according to organizational policies, provider protocols, and regulatory guidelines.
  • Schedule patient appointments, diagnostic testing, referrals, and follow-up visits as appropriate.
  • Provide patient education and reinforce provider care plans to support positive patient outcomes and patient satisfaction.
  • Accurately document all patient interactions, assessments, interventions, and care coordination activities within the electronic medical record.
  • Support the mission and standards of the organization through collaboration, professionalism, evidence-based practice, and commitment to quality patient care.
  • Maintain compliance with HIPAA, institutional policies, accreditation standards, and nursing best practices.
