Job description

Family Nurse Practitioner (FNP)

Connect Health + Wellness has an opeing for a Family Nurse Practitioner at our Bassett, VA location. 

Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our Federally Qualified Health Center sites in Bassett, Ridgeway and Patrick Springs and our Dental Clinic in Martinsville and Patrick Springs by promoting health, reducing health risk factors, and increasing access to medical dental and behavioral health services, primarily for the uninsured and underserved in our service area. Additionally, Connect Health + Wellness offers chronic disease self-management, medication assistance, and care coordination services to patients and residents within our service area.

Our Family Nurse Practitioners provide comprehensive, evidence-based, primary and preventive medical care in a team-based environment to a diverse patient population with a wide range of health conditions and concerns. Perform physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.

Pay Range: $95,000 to $120,000
