Position Overview

Compass Clinical Services is looking for a dedicated and experienced Licensed Mental Health Counselor to join our team in Elk Grove, California. This full-time remote position allows mental health professionals to deliver vital counseling and case management services to individuals within our community. As part of a Federally Qualified Health Center, you will play an essential role in their mental and emotional well-being, helping to address various bio-psychosocial needs.

Key Responsibilities
  • Conduct comprehensive assessments to evaluate patients' psychosocial needs, identifying appropriate treatment plans and referral options.
  • Provide individual and group counseling sessions, incorporating evidence-based therapeutic techniques tailored to the unique needs of each patient.
  • Perform effective case management by supporting patients in completing necessary forms for entitlements, supplemental insurance, and other benefits.
  • Advocate on behalf of patients when navigating interactions with community agencies and resources.
  • Maintain detailed and accurate patient records, ensuring that all documentation meets regulatory and organizational standards.
  • Coordinate essential transportation and social service resources for patients who require assistance accessing care.
  • Participate in team meetings and case conferences to discuss patient progress and share insights with colleagues, fostering a collaborative work atmosphere.
  • Identify and address educational needs of patients, delivering workshops or resources when necessary.
  • Perform targeted assessments and provide educational resources for prenatal and postpartum patients, focusing on mental wellness.
  • Collaborate with healthcare providers for referrals, maintaining a continuum of care for all patients.
  • Evaluate the unique needs of special populations, including marginalized and vulnerable groups, to enhance care delivery.
  • Provide HIV pre-test and post-test counseling, ensuring comprehensive support throughout the testing process.
  • Engage in follow-up contacts for missed appointments to reaffirm the importance of consistent care.
  • Contribute to Quality Improvement and Quality Assurance activities, identifying areas for enhancing services.
  • Ensure your work environment is clean, safe, and conducive to productivity, adapting as required by health regulations.
  • Perform additional duties as assigned or requested to support organizational needs.
Patient Population / Client Focus

Qualifications
  • A Bachelor's Degree in Social Work or a related field is required, with preference for candidates with advanced education.
  • Must hold a valid Licensed Mental Health Counselor (LMHC) certification or similar credential.
  • A minimum of two years of relevant experience working as a Social Worker is required, with five years preferred, reflecting strong capabilities in clinical practice.
  • Demonstrated proficiency with computer software applications including MS Word, Excel, and Outlook for efficient record-keeping and communication.
Preferred Experience
  • Experience utilizing telehealth platforms to deliver counseling services effectively is highly desirable in this role.
  • Familiarity with community resources and entitlement programs that support mental health care.
  • Proficiency in working within a Federally Qualified Health Center or community health context.
Compensation and Benefits

The competitive salary for this position ranges from USD 115,000 to 130,000 per year, commensurate with experience and qualifications.
