Centerville Post Acute is seeking a compassionate, licensed, and dedicated Physical Therapist (PT) to join our rehabilitation team. Our therapists play a vital role in helping residents regain strength, mobility, independence, and quality of life following illness, injury, or surgery. We are looking for a team-oriented professional who is passionate about providing exceptional patient-centered care in a skilled nursing and rehabilitation setting.
Position Summary
The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing skilled physical therapy services that maximize functional independence and improve overall quality of life. The Physical Therapist collaborates with physicians, nursing staff, and the interdisciplinary team to ensure the highest level of resident care.
Essential Responsibilities
  • Evaluate residents and develop individualized physical therapy treatment plans.
  • Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
  • Establish measurable treatment goals and modify care plans based on resident progress.
  • Educate residents, families, and caregivers on therapeutic exercises, safety techniques, and discharge recommendations.
  • Complete timely and accurate clinical documentation in accordance with facility, state, federal, and payer requirements.
  • Participate in interdisciplinary care planning meetings.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
  • Recommend adaptive equipment, assistive devices, and environmental modifications as appropriate.
  • Ensure compliance with Medicare, Medicaid, and rehabilitation documentation guidelines.
  • Maintain current knowledge of evidence-based physical therapy practices and regulatory standards.
Qualifications
  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the State of Ohio.
  • CPR certification required.
  • Minimum of 2 years of physical therapy experience preferred , with skilled nursing or post-acute care experience strongly preferred.
  • Excellent communication, organization, and clinical decision-making skills.
  • Experience with electronic medical records preferred.

  • Ability to motivate and encourage residents throughout the rehabilitation process.

Physical Requirements
  • Ability to stand, walk, bend, kneel, and assist residents throughout the workday.
  • Ability to lift, transfer, and support residents using appropriate equipment and safe patient handling techniques.
  • Ability to occasionally lift up to 50 pounds.
