Position Overview:
We are seeking a passionate and dedicated Licensed Registered Nurse (RN) to join our home health care team. As an RN with Celebrity Home Health, you will have the opportunity to make a positive impact in the lives of our patients by providing skilled nursing care, education, and support in their home environment.
Responsibilities:

• Conduct initial assessments and develop comprehensive care plans tailored to each patient's needs.
• Provide skilled nursing care, including wound care, medication management, and symptom management.
• Monitor patient progress and adjust care plans as needed in collaboration with the interdisciplinary team.
• Educate patients and their families about their conditions, medications, and self-care techniques.
• Coordinate care with other healthcare providers and community resources to ensure continuity of care.
• Document patient assessments, interventions, and outcomes accurately and timely in accordance with regulatory standards.

Qualifications:

• Current licensure as a Registered Nurse (RN) in the state of [Insert State].
