Caring Hospice Services is seeking a Social Worker

Choose your work location: Mashpee area

JOB DESCRIPTION

As Social Worker for Caring Hospice, you will provide quality hospice care, end-of-life, educational and grief support services. We are currently seeking a Social Worker to conduct hospice assessments and patient visits.

JOB REQUIREMENTS

  • Must be a Licensed Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) in the state of MA.

  • Minimum of 2 years of hospice experience preferred in hospice or oncology.

  • Candidate must have previous experience in conducting comprehensive assessments.

  • Ability to work effectively on an interdisciplinary team.

  • Prior experience making funeral arrangements; patient care planning, accessing community resources and discharge planning.

  • Ability to provide grief counseling/support.

  • Ability to foster open communication with patients, patient’s families, facility partners, staff and other healthcare providers

  • Must have reliable transportation, and be able to travel to where the patient resides.

Job Type: Part-time
