Social Worker/Care Companion

Carease Health - FL

We're seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team at Carease Health - FL, a pioneering organization committed to proactive measures for enhanced physical health and well-being. As a Social Worker/Care Companion, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

About the Role

In this part-time position, you'll have the opportunity to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment. Your primary responsibilities will include:

  • Visiting designated facilities in accordance with the schedule set with your supervisor

  • Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Documenting visit notes and maintaining detailed records of interactions and interventions after each visit

  • Collaborating with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Requirements

  • Associate Degree in Social Work, Psychology, Counseling, or another mental health-related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • 1 year of experience
