Work Environment

This full-time position is a remote role, providing team members the flexibility to work from home while still engaging in meaningful interactions with clients through virtual mediums. As a member of our multidisciplinary team within a community health clinic framework, you will collaborate with healthcare professionals to ensure comprehensive care for our clients.

Key Responsibilities

• Conduct detailed assessments of clients' psychosocial needs, focusing on emotional, mental, and social aspects while considering their unique life circumstances.
• Develop tailored care plans in partnership with clients and their families, ensuring that their individual goals, preferences, and cultural backgrounds are prioritized.
• Deliver counseling and emotional support, guiding clients through significant life transitions and challenges, while helping them build coping strategies.
• Advocate for clients, connecting them to vital community resources and services that support their mental health and overall well-being.
• Collaborate closely with healthcare teams, including nurses and caregivers, to enhance the quality of care delivered to clients.
• Regularly engage in monitoring visits, assessing clients’ living conditions, and addressing emerging concerns to ensure a safe environment.
• Facilitate effective communication among families involved in care, addressing conflicts and promoting harmony in support of the client.
• Provide education to clients and families regarding available resources, support groups, and programs to enrich their quality of life and social engagement.
• Maintain precise and up-to-date records documenting assessments, care plans, client interactions, and progress toward goals.
• Stay informed regarding current social work practices and ensure compliance with relevant ethical standards and regulations.

Patient Population / Client Focus

The Social Worker in this role primarily serves seniors and their families, recognizing the distinct challenges faced by this demographic. You will provide empathetic support, helping clients navigate the complexities of aging while also addressing mental health concerns. Your involvement will directly impact the clients' ability to manage anxiety and depression, enhancing their overall quality of life.

Qualifications

• Master's degree in Social Work from an accredited program, with a current and valid Illinois state license as a Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in social work, particularly within community health or geriatric settings.
• Comprehensive understanding of aging-related issues, social work principles, and available community resources pertinent to seniors.
• Exceptional skills in assessment, counseling, and problem-solving are essential.
• A compassionate approach with the ability to establish trust and rapport with clients and their families.

• Proficiency in electronic health records (EHR) systems and other relevant software applications.

Preferred Experience

Experience working in community health clinics or geriatric populations is highly preferred. Familiarity with telehealth delivery methods will be beneficial in this remote role, enhancing engagement with clients and their families.

Compensation and Benefits

The salary for this full-time role ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications. We offer a comprehensive benefits package, including:
