Job description
Vous etes un pharmacien a la recherche d'un poste a temps partiel dans un CHSLD avec horaire flexible . Ce poste est pour vous . La personne sera rattachee a la pharmacie Proxim Jean-Francois Morin, Benoit Morin et Stephane Morin, associee au Groupe Bergeron Dupuis & Associes, situee a Montreal, mais votre poste serait en CHSLD prive a Saint-Georges de Beauce.
Le mandat est de 6.4 heures par semaine ou plus, selon vos disponibilites, et vous permettra d'etre proche des patients et de leur assurer des soins personnalises. Horaire flexible, a vous de decider si vous preferez faire les heures de jour, soir ou fin de semaine. Votre role serait de : Repondre aux consultations pharmacologiques des medicaments entourant les admissions et retours d'hopitaux; Evaluer les situations de polypharmacie; Favoriser l'utilisation optimale des medicaments dans un contexte de milieu de vie; Noter au dossier pharmacologique d'un resident toute intervention pharmaceutique; Se familiariser avec le programme OPUS-AP et PEPS; Assister a divers comites cliniques tel que les equipes interdisciplinaires
