Join Anthem Hospice as a Medical Social Worker (MSW) – Oklahoma City, OK

At Anthem Hospice, we believe exceptional hospice care means supporting the whole person and their family. We are seeking a compassionate Medical Social Worker (MSW) to join our Oklahoma City team and provide meaningful emotional, psychosocial, and resource support to patients and families during one of life’s most important journeys.

As part of our interdisciplinary hospice team, you’ll work closely with patients, families, nurses, physicians, chaplains, and other care team members to identify needs, provide counseling and support, connect families with community resources, and help ensure each patient receives compassionate, individualized end-of-life care.

Essential Functions
  • Assesses patient/family needsin a timely mannerupon receiving patient admission;recognizingthepatient’sabilitysupportsystemscapability and thepatientsexpressedwishes.
  • Assessespatient’senvironmentandfinancial needs and makesappropriate referralstoidentifiedcommunityresources.
  • Developsaplanofcareincollaborationwithphysician’steampatientandfamilytomeet identified needsandmaximize positive outcomesinatimelymanner.
  • Activelyparticipatesinmultidisciplinaryteamconferences anddocumentsoutcomes asneeded;assistingtheteaminunderstandingthesocialandemotionalfactorsrelatedtothepatient'shealthproblems.
  • Maintains documentationinpatientsrecordper internal regulatory and professional standards.
  • Establishesshortandlong-termgoals andcompletes themaccordingto thepatientsplan of care.
  • Maintainsvisitproductivitytomeetpatientneedsandteamresponsibility.
  • Monitorsobserveandevaluatechangesand progress inpatients’condition and environmentand reportschangesprogressor lack ofprogresstophysicianand/ornursecase manager.
  • Involvespatientandfamily/significantotherintheplanofcarewhilerevisinganddocumenting strategies/interventiontoaccommodatechangingneedsand/orbarriers.
  • Actsas keysourceinpatientsituationssuchas:ineffective patient/familycopinganddecision‑makingadvancedirectiveslong‑termorassistedlivingplacementsubstanceabuseabuse/neglect and/orbereavement.
  • Demonstratesteambuildingskills.Servesasrolemodel/resourceforotherteammembers.
  • Assumesresponsibilityandaccountabilityforindividualsocialworkjudgmentsandactions.Identifiesstrengths/weaknessesinclinical practice and makesnecessary adjustments.
  • Participatesincost containmentmeasuresincludingutilizingsuppliesefficiently;accuratelyrecordingallcharges; executing efficientpatientflow;utilizingworktime productivity; andidentifyingactual potential problems.
  • CommunicateswithURcoordinatorforanyauthorizationsrequiredperinsurancerequirements.Maintains strict confidentiality of all Carroll Hospice/CHG system matters including patient physicianAssociate and financial information.
  • Appropriatelyrepresentstheagencyatmeetingsbothinsideandoutsidethefacility.
  • Effectivelyutilizesproperchannelsofcommunication withcolleaguesphysicianspatientsfamiliesvisitorsand outsidecontacts to ensureclear concise flow of patient information.
  • Assumesresponsibility for continuous education and licensure requirements through participation in educationalprograms,currentliteraturein-servicesmeetings workshopsetc.
  • Assumes responsibilityfor professional growth by keepingabreast of changing hospice/homecare standards and changing trends in the social work professionand attends andparticipatesin mandatory staff meetingsandin-servicesaccording to departmentalstandards.

Qualifications

Licenseorcertification:

  • LMSW – Certified Clinical Social Worker
