Key Responsibilities
  • Conduct thorough mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments, to accurately identify patient needs.
  • Develop tailored treatment plans in collaboration with patients that align with their goals, cultural backgrounds, and personal circumstances.
  • Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) in individual, group, family, and couple counseling sessions.
  • Respond to patient crises by providing urgent intervention services and coordinating care with interdisciplinary teams to ensure continuity and quality of care.
  • Maintain comprehensive and precise clinical documentation, including progress notes and discharge summaries, while adhering to regulatory and clinic standards.
  • Teach psychoeducation tailored to individual patient needs, equipping patients and families with the knowledge and tools to manage their mental health effectively.
  • Work together with a diverse multidisciplinary team including psychiatrists, psychologists, nurses, and case managers to promote an integrated care model for patients.
  • Regularly assess treatment outcomes, making data-informed adjustments to treatment approaches and methodologies as necessary.
  • Stay abreast of current research, best practices, and developments in mental health, ensuring the application of the latest knowledge in clinical practice.
  • Uphold ethical guidelines and prioritize maintaining the confidentiality and security of patient information.
Patient Population / Client Focus

This position serves diverse patient populations within a hospice setting, characterized by unique mental health challenges often related to terminal illnesses. You will engage with patients and their families at sensitive times, requiring a deep sense of empathy and cultural competence. Your work will significantly impact their emotional and psychological support during these crucial moments of life.

Qualifications
  • A Master’s degree in Social Work (MSW) is required.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • Ability to work independently with strong self-management skills while also being an effective team player within a multidisciplinary setting.
  • Exceptional interpersonal and communication skills, enabling rapport-building with patients from a multitude of cultural backgrounds.
Preferred Experience
  • Previous experience in a hospice, palliative care, or similar healthcare environment is highly desirable.
  • Familiarity with electronic health record (EHR) systems and commitment to maintaining accurate patient records.
  • Understanding of state-specific regulations concerning social work practices within hospice care.
Compensation and Benefits
  • The annual base salary range for this position is between $70,000 and $84,000, depending on experience and qualifications.
