Work Environment

This position is entirely remote, enabling clinicians to work from the comfort of their homes while effectively engaging with patients via telehealth methods. Key Responsibilities
  • Conduct thorough mental health assessments, including psychosocial evaluations and risk assessments, to understand each patient's unique situation.
  • Collaborate closely with patients to create customized treatment plans that take into account their individual needs, goals, and cultural backgrounds.
  • Implement evidence-based therapeutic interventions in various formats, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), tailored to the patient's circumstances.
  • Provide timely crisis intervention services to patients experiencing acute distress, coordinating with multidisciplinary teams to maintain continuity of care.
  • Maintain comprehensive clinical documentation, including progress notes and treatment plans, to ensure compliance with regulatory guidelines and clinic policies.
  • Offer psychoeducation to patients and their families, equipping them with skills and knowledge to manage their mental health conditions.
  • Conduct regular outcome assessments to gauge treatment effectiveness and adjust treatment plans as necessary to optimize patient outcomes.
  • Stay updated on the latest research and best practices in mental health to enhance clinical practice and patient care.
  • Adhere strictly to ethical guidelines and maintain patient confidentiality in all communications.
Patient Population / Client Focus

This role focuses primarily on providing care for patients within the hospice setting. You will work with individuals facing serious, life-limiting conditions, offering them emotional support and guidance. Collaborating with families, you will address concerns around end-of-life decisions, grief, and bereavement, ensuring they have the comprehensive support they need throughout this challenging journey.

Qualifications
  • Master's degree in Social Work (MSW) is required.
  • An active State license as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • Demonstrated ability to work both independently and collaboratively in a team environment.
  • Exceptional interpersonal and communication skills, especially in building rapport with patients from a variety of cultural backgrounds.
Preferred Experience

While not required, experience in hospice, palliative care, or working with terminally ill patients is highly valued. Familiarity with telehealth platforms and electronic health record (EHR) systems will also contribute to a smoother onboarding process.

Compensation and Benefits
