Work Environment

Key Responsibilities

• Conduct thorough examinations and evaluations of patients to assess functional limitations and disabilities, leading to individualized treatment planning.

• Implement evidence-based treatment protocols that align with patient presentations and remain adaptable to changes in the healthcare environment.

• Document all patient care activities accurately and timely, including initial evaluations, daily encounter notes, interim progress reports, and discharge summaries.

• Collaborate with interdisciplinary teams to provide comprehensive patient care, recognizing when to modify treatment plans or consult with other healthcare professionals.

• Complete discharge planning and ensure follow-up care is in place for optimal patient outcomes.

• Participate in ongoing professional development and maintain all necessary training to be compliant with state, local, and federal regulations.

• Engage actively in facility meetings and contribute to initiatives that focus on improving patient access and healthcare quality.

Patient Population / Client Focus

Our Physical Therapists work primarily with patients who experience a variety of orthopedic conditions requiring rehabilitation and strength training. Typical cases include post-surgical recovery, sports-related injuries, and chronic pain management. Our focus is on delivering personalized care in a supportive environment that encourages patient engagement and progress.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program is required.

• Current state licensure as a DPT.

• A minimum of two years of experience in outpatient orthopedics is essential.

• Current CPR certification.

Preferred Experience

Experience in home care settings is a plus, as it enables the Physical Therapist to adapt their skills and knowledge to varied environments and health challenges faced by patients outside traditional clinical spaces.
