HCBS Coordinator
Acacia Care LLC | Phoenix, AZ
Full-Time | $19-$25/hour
About the Role
Acacia Care is seeking a compassionate and organized HCBS Coordinator to support individuals with developmental disabilities while overseeing staff, service delivery, and program operations.
Work Schedule: This position is on-site during training, with the opportunity to transition to a hybrid schedule upon successful completion of training, based on performance, proficiency, and business needs.
Key Responsibilities
  • Recruit, onboard, and supervise Direct Support Professionals (DSPs)
  • Coordinate and monitor HCBS services
  • Support members and families
  • Ensure compliance with DDD, HCBS, and EVV requirements
  • Train and coach staff
  • Assist with scheduling and daily program operations
  • Conduct service monitoring and occasional travel as needed
Qualifications
Required
  • Bachelor's degree in Human Services, Social Work, Education, Healthcare Administration, or a related field; or an equivalent combination of education and experience
  • Experience coordinating services, supervising staff, or supporting individuals with developmental disabilities
Preferred
  • 3+ years of experience in developmental disability services, HCBS, DDD, case management, program coordination, or a leadership role
  • DDD, HCBS, and EVV experience
  • Strong Microsoft Office and Excel skills
  • Bilingual Spanish

Requirements
  • Valid driver's license and reliable transportation
  • Ability to work occasional evenings, weekends, or holidays
  • Successful completion of background screening
  • Ability to obtain a Level 1 Fingerprint Clearance Card
