- Life $25,000, Disability & Dismemberment $25,000, Dependent Life $2,500 spouse / $1,000 child, Extended Health 100% for Hospital / Ambulance / Travel, Prescription Drugs 80% $50,000 per year max, Paramedicals - 80% $750 per practitioner type, Vision Wear - $200 per 24 months 1 eye exam per 24 months Max $75, Dental - 80% Basic /50% Major $2000 per year combined.
