Work Environment

Key Responsibilities
  • Conduct comprehensive assessments to evaluate clients’ social, emotional, and environmental challenges, ensuring the development of personalized care plans.
  • Design and implement service plans that address identified issues, incorporating client goals and progress tracking.
  • Provide individualized counseling and therapeutic support to clients and families, facilitating coping strategies and resilience in facing life challenges.
  • Engage in crisis intervention where necessary, conducting risk assessments and safety planning to protect client welfare.
  • Collaborate with community agencies and services to coordinate comprehensive care and advocate effectively on behalf of clients.
  • Participate actively in community outreach initiatives, promoting agency services and raising awareness about mental health and social challenges in the community.
  • Attend and contribute to case conferences, professional training sessions, and team meetings to enhance service delivery and foster professional growth.
Patient Population / Client Focus

The clients you will work with span a diverse range of backgrounds and experiences. Many suffer from significant life stressors, including mental health disorders and the socio-economic complexities associated with issues like homelessness and domestic violence. Your role will be pivotal in helping these individuals navigate their challenges, offering them vital support and resources to improve their overall quality of life.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.
  • Strong foundational knowledge of trauma-informed care and the social determinants of health impacting community wellness.
  • Excellent communication and interpersonal skills, with a demonstrated ability to work effectively with diverse populations.
  • Ability to employ problem-solving skills to navigate complex client needs and challenges effectively.
Preferred Experience
  • Experience working in a social service agency, hospice center, or related field is highly desirable.
  • Familiarity with community resources and services available to at-risk populations is a plus.
  • Prior experience conducting assessments, developing service plans, and providing direct therapeutic interventions.
