Job description

Pride Health is hiring a Licensed Social Worker to support our client’s Healthcare Facility in Waukesha, WI 53189. This is a 3-month contract assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Job Name: Licensed Social Worker
Location: Waukesha, WI 53189
Duration: 3+ months Contract (Possibility of extension)
Pay rate: $34-$37/hr. On W2 (Negotiable - Pay Rate is based on experience)
Schedule: Shift start times at 8:00 AM or 12:00 PM (8.5-hour shifts - including meal break)/ Weekday and weekend coverage required. (40 hrs./ wk.)
Job Responsibilities:

  1. Provide direct social work and case management services to patients and their families throughout the hospital stay.
  2. Conduct comprehensive psychosocial assessments to identify patient needs, barriers to care, and appropriate interventions.
  3. Deliver crisis intervention, emotional support, and brief counseling for patients and families experiencing acute or complex situations.
  4. Demonstrate a strong understanding of mental health diagnoses and incorporate appropriate interventions into patient care plans.
  5. Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care and discharge planning.
  6. Identify patient resource needs and connect individuals with appropriate community programs, services, and support agencies.
  7. Coordinate referrals and facilitate communication between the hospital and community providers to ensure continuity of care.
  8. Arrange and facilitate interdisciplinary care conferences and family meetings as needed.
  9. Respond to patient, family, staff, and community information and referral requests in a timely manner.
  10. Advocate for patients by helping address psychosocial barriers and promoting access to appropriate services.
  11. Assist patients and families with problem-solving, coping strategies, and wellness-focused planning.
  12. Maintain accurate, timely, and confidential documentation in accordance with hospital policies and regulatory requirements.


Qualifications:

  • Master’s degree in social work, or higher required.
  • Licensed to practice in the State of Wisconsin.
  • Previous hospital social work experience preferred.
  • Prior Emergency Department experience highly preferred.
