Job Overview

The Licensed Social Worker (LSW) provides comprehensive psychosocial assessments, counseling, and continuum of care planning within the award-winning outpatient renal program at Trinitas Regional Medical Center. Operating within our CMS 5-star rated and 5-Diamond Patient Safety designated facilities, this professional serves as a vital patient advocate and key member of the interdisciplinary care team. The role focuses on helping individuals and families navigate end-stage renal disease, maximizing medical compliance, securing financial entitlements, and fostering a compassionate, family-centered environment.

    Qualifications

    Required:

    • Master of Social Work (MSW) degree from an accredited educational institution.
    • Active Licensed Social Worker (LSW) credential in accordance with New Jersey state regulations.
    • Excellent documentation, communication, and interpersonal skills to handle constant typing, talking, and collaborative charting.

    Preferred:

    • Prior social work experience in a hospital or healthcare setting.
    • Familiarity with nephrology, end-stage renal disease (ESRD) regulations, and Medicare/Medicaid entitlements.

    Scheduling Requirements

    • Part-Time (PT) - Day
    • Shift details and hours will be discussed during the interview process.

    Essential Functions

    • Care of Patient: Implements patient- and family-centered care; completes mandatory psychosocial assessments for all newly diagnosed chronic renal failure patients within 30 days of admission.
    • Interdisciplinary Collaboration: Actively participates in interdisciplinary patient care rounds and care planning alongside physicians, nurses, care managers, and dietitians.
    • Continuum of Care: Secures financial entitlements, coordinates transit services, arranges home care needs, and refers patients to vocational rehab or community agencies.
    • Patient Advocacy & Support: Guides patients through Advance Directives, assists with temporary or permanent transfers to other dialysis units, and addresses non-compliance issues.
    • Education & Support Groups: Participates in Renal Support Groups; conducts annual in-service education for nursing and renal staff regarding patient entitlement programs.
    • Quality & Compliance: Utilizes continuous quality improvement (CQI) principles to improve department functions; ensures accurate quarterly documentation on all outpatients.
    • Core Competencies: Maintains strict compliance with risk reporting, healthcare error tracking, infant abduction prevention, and mandatory abuse/neglect referral protocols.
