Position Title: Family Nurse Practitioner (FNP) Location: NYMVCare – Multiple Locations (Brooklyn & Queens) Department: Medical Services Reports To: Medical Director / Operations Manager Schedule: Full-Time / Part-Time (Hours Vary by Location)

Position Overview

At NYMVCare, the Nurse Practitioner plays a critical role in delivering high-quality, compassionate, and patient-centered care in a fast-paced, team-based clinical environment. Working closely with our physicians, specialists, and support staff, the NP is responsible for evaluating, diagnosing, treating, and managing acute and chronic medical conditions, while maintaining the highest standards of clinical excellence and regulatory compliance.

Key Responsibilities

Conduct comprehensive physical exams, assessments, and medical histories

Diagnose and treat a variety of common acute and chronic conditions

Order and interpret diagnostic tests and imaging when appropriate

Develop and implement patient care plans, including follow-ups and referrals

Prescribe medications and therapies in accordance with state regulations

Provide patient education and counseling regarding disease prevention, treatment adherence, and wellness

Coordinate care with other providers including specialists, therapists, and ancillary services

Accurately document all patient encounters and updates in the Electronic Medical Record (EMR)

Participate in clinical quality initiatives and case reviews as needed

Maintain a strong working knowledge of health insurance guidelines, coding, and regulatory updates

Uphold professional standards, including HIPAA compliance and infection control protocols

Required Qualifications

Active and unrestricted New York State Nurse Practitioner license

National certification (ANCC or AANP) in Family, Adult-Gerontology, or Internal Medicine

DEA registration or eligibility

Minimum of 1 year of primary care or internal medicine experience preferred (new grads considered based on clinical experience and training)

Proficiency in using EMR systems (experience with PrognoCIS, eClinicalWorks, or similar systems a plus)

Strong interpersonal, communication, and organizational skills

Bilingual proficiency a plus but not required (English required; Spanish, Arabic, or Russian preferred)

Work Environment & Expectations

Multi-specialty outpatient setting with diverse patient populations

Locations may rotate; must be flexible to work across Brooklyn and Queens clinics as scheduled

Collaboration with medical, physical therapy, and administrative teams required

Professional attire and demeanor expected

Must be punctual, responsive, and able to manage high patient volumes without compromising care quality
