Job Summary:

Carease Health - OH, a nationwide preventative healthcare company, is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner, you will play a vital role in our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to minimize hospitalizations and elevate patient care standards.

Responsibilities:

  • Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation

  • Build strong relationships with the facility, maintaining a patient-centered approach and being a collaborative team player

  • Stay up-to-date with the latest clinical guidelines and best practices in Geriatric care and Internal Medicine

  • Conduct comprehensive assessments, develop treatment plans, and provide education to residents and families

  • Collaborate with facility staff, including physicians, nurses, and other healthcare professionals to ensure seamless care coordination

  • Identify and manage chronic conditions, and provide chronic care management services to residents

  • Utilize Remote Patient Monitoring (RPM) technology to continuously monitor vital signs and detect abnormalities

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in:

    • Geriatric care

    • Documentation review

  • Excellent communication, collaboration, and teamwork skills
