Job description

Summary
The primary function of this position is to provide maternal case management by assisting the Women's Health Nurse Case Manager and the Maternal Child Health (MCH) Grant Project. Lead Physician to assist in the coordination of care of CCHCF pregnant patients with diabetes and/or hypertensive related complications of pregnancy throughout their pregnancies and into the postpartum period.
A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

  • Works with the Women's Health Nurse Case Manager to identify a panel of pregnant patients who have diabetes and/or hypertensive diseases complicating their pregnancies.
  • Works closely with patients, offering weekly in person and/or remote contact to ensure patients are checking their home blood sugars, blood pressures, and attending clinic appointments.
  • Works with the Women's Health Nurse Care Manager and the Maternal Child Grant Physician Project Lead to assist and manage the Postpartum Women's Wellness Program which will require organizing and managing monthly meetings, scheduling and organizing outside consultants.
  • Provides patient education on the use of home blood sugar and blood pressure monitoring. Maintains and provides appropriate patient education material.
  • Assist in maintaining a SCOB (special care obstetrics) clinic rooster and scheduling SCOB clinic appointments by phone or in person.

Background Investigation Requirement:
This position requires the successful completion of a federal background investigation as a condition of employment.

Qualifications
To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.
MINIMUM QUALIFICATIONS:
GS06: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Working with Nurse Case Manager, working with patients, managing Postpartum Woman's Wellness Program, attending staff meetings, collecting and tracking data, providing patient education, maintaining appropriate patient education material, and identifying pregnant patients who have diabetes and/or hypertensive diseases.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).

Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee. IHS may offer newly-appointed Federal employees credit for their directly related previous non-federal experience or active duty uniformed military service. This credited service can be used in determining the rate at which they earn annual leave.
