Job description

Capital Cardiology Associates is seeking a dedicated and experienced full-time Cardiac Program Access Coordinator to join our team. This critical role is the operational link ensuring seamless patient entry into our specialized cardiac programs. You will manage the entire patient access and enrollment process, bridging the gap between providers, clinical teams, scheduling, and the revenue cycle to eliminate financial barriers and delays in care.

Responsibilities include but not limited to:

Patient Enrollment & Coordination

  • Serve as the primary access manager for specialized cardiac programs, including MCOT, HealthSnap, Remote Patient Monitoring, Cleerly Cardiac Imaging, and Pritikin Intensive Cardiac Rehabilitation.
  • Thoroughly review program orders from providers and confirm all patient eligibility requirements and program prerequisites are met.
  • Coordinate necessary enrollment steps directly with clinical teams to ensure readiness for participation.
  • Track program referrals, monitor enrollment status, and actively follow up on any incomplete steps or required documentation.
  • Communicate patient program readiness effectively to scheduling teams.

Insurance & Billing Management

  • Conduct comprehensive reviews of insurance benefits related to program participation.
  • Identify and calculate patient financial responsibilities, including deductibles, coinsurance, and copays.
  • Provide clear, estimated out-of-pocket costs to patients when available to ensure financial transparency.
  • Maintain accurate and timely documentation of all patient communication within the medical record.

Required Qualifications:

  • Current and valid LPN license in New York State.
  • Strong knowledge of current insurance guidelines and prior authorization requirements.
  • Proficiency in Electronic Medical Record systems and fundamental computer skills.
  • Exceptional organizational skills, with the ability to effectively manage multiple tasks and competing priorities.
  • Demonstrated professional, reliable, and team-oriented attitude.
  • Previous professional experience within a cardiology is preferred.
