Job description

BAYADA Home Health Care is looking for their next Division Director, Clinical Strategy, Innovation, and Care Delivery in our Skilled Nursing practice.

This is a remote position with travel expected at about 25%. Our ideal candidate will be located within New Jersey, Delaware, Pennsylvania and surrounding areas.

In this highly visible executive role, you’ll partner directly with the Practice President and senior leadership to shape the future of care delivery—driving clinical excellence, advancing innovative care models, and improving outcomes for patients across the continuum.

What You’ll Do

Drive Innovation in Care Delivery

  • Design and implement forward-thinking care models and population health strategies
  • Leverage technology, data, and emerging tools (AI, automation) to enhance care and efficiency
  • Lead transformation initiatives in response to evolving healthcare delivery and payment models

Elevate Clinical Quality & Outcomes

  • Ensure delivery of high-quality, evidence-based care with industry-leading outcomes
  • Advance performance across key metrics such as patient satisfaction, quality ratings, and value-based measures
  • Maintain excellence in regulatory compliance and accreditation standards

Build and Inspire High-Performing Teams

  • Recruit, mentor, and develop clinical leaders and teams
  • Establish best-in-class training, education, and professional development programs
  • Foster collaboration across clinical, operational, and corporate teams

Shape the Future of Value-Based Care

  • Partner with payers and internal stakeholders to expand value-based care initiatives
  • Optimize clinical and financial performance through effective resource management
  • Support revenue integrity through alignment with clinical documentation, coding, and compliance

Leverage Data to Drive Decisions

  • Use analytics and insights to guide strategy, measure success, and identify opportunities
  • Enhance reporting capabilities and performance transparency across the organization

Qualifications

Education & Licensure

  • Bachelor’s Degree in Nursing (required); Master’s degree in Nursing or related clinical field (preferred).
  • Active Registered Nurse (RN) license in good standing in the state of practice.

Experience, Skillset and Abilities

  • Minimum of 10 years of clinical leadership experience in home health, skilled nursing, or related healthcare settings, both adult and pediatric experience required.
  • Demonstrated success in clinical operations, multidisciplinary team leadership, and care delivery transformation.
  • Experience with value-based care models and healthcare reimbursement structures.
  • Deep knowledge of acute and post-acute care delivery systems.
  • Expertise in clinical quality metrics, reporting, and performance improvement.
  • Strong leadership, communication, and executive presentation skills.
  • Proven ability to influence, collaborate, and lead across complex organizations.
  • Ability to travel about 25%.
