  • Engage directly with insurance companies to manage pre-certification processes and ongoing utilization management, ensuring clients receive timely support.
  • Participate in intricate discharge planning and aftercare solutions, focusing on seamless transitions for patients moving from hospice care to other settings or home-based support systems.
  • Provide therapeutic meal support, guiding clients to model and sustain a healthy relationship with food, addressing both psychological and nutritional needs.
  • Deliver empathetic, affirming, and individualized care, grounded in an understanding of the clients’ intersecting identities and life circumstances.
  • Maintain meticulous records of client interactions, progress notes, and treatment plans in compliance with ethical and legal standards.
Patient Population / Client Focus

This role primarily supports older adults dealing with complex health issues in a hospice setting. Our clients may face emotional, social, and psychological challenges associated with terminal illness, necessitating a provider who is attuned to their unique needs and preferences. Qualifications
  • A Master's degree in Social Work from an accredited academic institution.
  • A current and unrestricted LCSW license within the state of Maryland is mandatory.
