Remote Registered Nurse, CDCES (Contracted): Compact / IL, MI, NY

Location: Remote — United States (Compact RN License Required; additional licensure in IL, MI, and/or NY highly preferred)

THE HIGHLIGHTS
  • Anticipated Start Date: Spring/Summer 2026 (TBD)
  • Role Type: Contracted, 1099
  • Payment Terms: Net 30
  • Licensure: Compact (Multi-State) RN license required. Additional active licensure in IL, MI, and/or NY is highly preferred.
  • Certification: CDCES (Certified Diabetes Care and Education Specialist) required.
  • Schedule: 1099 flexibility; you set your own hours; ideally 20+ hours of weekly availability.
    • Current Demand: Monday–Friday, 9 a.m.–6 p.m. ET
    • Requirement: Provide availability for the next two months by the 15th of each month.
  • Compensation: $20.00 per 20-minute clinical encounter + $30.00/hour for non-clinical time; budget for an average of $40/hr (see Compensation & Payment Terms below).
  • Remote Requirements: Must reside in the United States with consistent access to a clean, quiet workspace; solid internet connection; and proficient technical experience.

Perry Health is rethinking chronic care through a fully remote, continuous care model. We provide expert diabetes education and clinical management for Medicare patients, delivered by a comprehensive team of specialized Registered Nurses, Registered Dietitians, and Accountability Coaches.

Our model integrates high-level clinical expertise with real-time data from cellular-connected glucometers and advanced CGM management. By bridging the gap between physician visits with proactive, data-driven support, we deliver life-changing outcomes for our members.


ABOUT THE ROLE

We are seeking an experienced Registered Nurse (RN) and Certified Diabetes Care and Education Specialist (CDCES) passionate about remote care delivery. In this role, you will lead diabetes education and management, focusing on medical intervention and acute data management for our members. You will partner with a multidisciplinary care team to ensure members receive expert clinical attention and education to lead healthier lives.


KEY RESPONSIBILITIES
  • Provide call-based care and clinical management to patients in our Remote Patient Monitoring (RPM) program.
  • Triage acute blood glucose readings and monitor real-time data trends via our proprietary EHR.
  • Support members with medication and insulin interventions to ensure safe and effective diabetes management.
  • Deliver empathic, high-touch care that alleviates the emotional burden of managing a chronic disease.
  • Collaborate with the broader care team — including RDs and Accountability Coaches — to reinforce clinical goals and SMART goal progression.
  • Adhere to Perry Health policies, procedures, and standard operating procedures to ensure ongoing clinical quality and compliance.

QUALIFICATIONS
  • Education: ADN or BSN in Nursing required.
  • License: Valid, unencumbered Compact (Multi-State) RN license required. Additional active licensure in IL, MI, and/or NY is highly preferred.
  • Certification: CDCES (Certified Diabetes Care and Education Specialist) required.
  • Experience: Minimum 3+ years of experience managing patients with diabetes.
  • Knowledge: Familiarity with continuous glucose monitors (CGMs) and behavior change theory is a plus.
  • Skills: Motivated learner, willing to acquire proficiency with the necessary technology (computers, software applications, phone systems); experience or training in Motivational Interviewing to support sustainable behavior change is a plus.
  • Traits: Creative thinking, an eye for detail, ability to work independently, a positive attitude, and the ability to excel in a fast-paced atmosphere while collaborating with diverse groups at all levels of the organization.
  • Language: Spanish-speaking highly preferred.

COMPENSATION & PAYMENT TERMS

Contractors are paid on a monthly basis following Net 30 payment terms.

  • Clinical Pay: $20.00 per 20-minute clinical encounter. An encounter is defined as a successful call with a member, primarily spent in live conversation, with chart review time also counting toward this total.
    • Example: Clinical encounters are scheduled in 30-minute intervals. Completing two encounters within an hour that each meet the engagement-time requirement pays a total of $40 for that hour.
  • Non-Clinical Pay: $30.00/hour flat rate for required meetings and training.
