Job description

Position: Cancer Action Case Manager – Part Time Position Description: Cancer Action is seeking to hire a compassionate professional to be a part-time case manager. Cancer Action is a local nonprofit providing practical and financial services and emotional support to cancer patients and their families living in the Kansas City metro area. Case managers help reduce distress and improve well-being of cancer patients and their caregivers by arranging services to meet physical and financial needs and offer emotional support to the patient and family throughout their personal cancer journey.

They are the anchors of the programs and services offered by Cancer Action. This part-time position (3 days/week & occasionally PRN) will provide case management services through the Cancer Action offices in Overland Park, KS and Independence, MO Specific Responsibilities: Responsible for assessing client/family needs and coordinating agency services. Providing compassionate, equitable and timely care and support to all clients.

Assist clients with the application and other required paperwork for the delivery of agency services. Collaborate with health care partners, referral sources to arrange needed services and help meet the ongoing needs of clients. Provide emotional support, education and guidance to help the client navigate their personal cancer journey.

Responsible for ongoing client follow-up, identifying new needs and assuring clients' needs have been met to the best of the agency's abilities. Provide ongoing case management and emotional support to clients being served through the assigned office. Collaborate with other Cancer Action case managers to ensure smooth and seamless delivery of services.

Provide referrals and information to clients regarding community resources, Continue to be educated and knowledgeable of current community resources. Provide bereavement support to families as necessary. Help organize and maintain storage rooms, wig rooms and performing other tasks necessary in the efficient delivery of client services.

Maintain accurate and up-to-date case records, service delivery reports and other paperwork necessary to efficiently provide care to clients. Performs additional related duties as assigned by supervisor or Director. These may include helping with special events, special projects etc.

Qualifications: Minimum requirement is a bachelor's degree in social work or related field with at least 2 years' experience. MSW or related master's degree preferred. Health care experience, bilingual is a plus.

Prefer experience in working with diverse populations. Ability to work both independently and as a part of a team. Ability to communicate well and represent the mission of Cancer Action to clients, volunteers, and the community.

Ability to provide compassionate, professional care to agency clients. Effective communication skills, both verbal and written. Strong problem-solving skills.

Excellent organizational skills and ability to plan, organize, multitask and prioritize workload. Good computer skills necessary for client data reporting and service delivery tracking efficiently. Knowledge of local, national and government resources, health care and/or issues impacting those with cancer and impact of serious illness helpful.

Ability to maintain confidentiality and uphold professional ethical standards, agency policies and procedures in maintaining a respectful work environment. Able to drive and have a reliable vehicle. Ability to perform physical activity related to position including moderate lifting of equipment and supplies, bending and carrying weight to/from vehicle, storage room, etc.
