Job description

Pharmacist

Provides professional pharmacy services to the community. Reviews, interprets, and dispenses medications, drugs, and other pharmaceuticals as prescribed by physicians/providers according to professional standards, state and federal laws and regulations. Counsels patients on medication use and provides drug information to patients and licensed personnel. Performs monthly medication regimen reviews (MRR) and prepares quarterly summaries of trends and findings. Attends quarterly Continuous Quality Improvement (CQI) meetings. Provides drug information to medical and nursing staff. Provides drug safety education in-services. Supports the Director of Nursing as necessary. Meets state and federal laws and regulations. Reviews and interprets medication orders. Reviews medication profiles to prevent adverse drug reactions, therapeutic duplications, dosing errors, and drug-drug interactions. Monitors therapeutic treatments and relevant lab values and parameters. Adheres to protocols and performs therapeutic interchanges. Supervises ancillary staff such as pharmacy technicians and/or clerks. Performs projects as assigned necessary for a functional and efficient pharmacy. Performs clinical interventions and monitors therapies. Participates in clinical programs and accesses patient-specific data when performing medication verification. Demonstrates knowledge and understanding of therapeutic drug monitoring. Performs patient care in a manner that promotes the best outcomes for patients by minimizing potential medication errors. Demonstrates knowledge and proficiency in renal function calculation and dosing adjustment. Verifies and prepares medications by reviewing and interpreting physician and provider orders for therapeutic appropriateness. Dispenses medication by compounding, packing, and labeling appropriately. Organizes and directs ancillary staff workflow, verifying their work, and validating order entries. Conducts outdate inspections of all medication areas. Complies with federal laws and regulations set by relevant authorities. Maintains a safe and clean working environment by adhering to policies and procedures. Attends Continuing Education (CE) to maintain pharmaceutical knowledge. Provides professional service to all customers, colleagues, and coworkers. Gains proficiency with CPOE systems and medication error reporting systems. Communicates clearly and appropriately with medical staff, nursing staff, and the public. Documents clinical interventions and monitoring when applicable. Maintains department goals in compliance with relevant laws and regulations. Demonstrates knowledge of current approved pharmacy protocols. Understands daily workflow and responsibilities. Performs other duties as assigned. Adheres to dress code, ensuring neat appearance and timely arrival for shifts. Completes annual education requirements, attends staff meetings, and performs departmental in-service training. Maintains patient/resident and employee confidentiality. Prioritizes tasks while adapting to changing departmental needs. Represents the organization in a positive and professional manner. Maintains good rapport and cooperative working relationships with all staff. Complies with organizational policies on ethical practices. Communicates the mission, ethics, and goals of the facility.
