• Conduct ongoing assessments to identify the mental health needs of patients and formulate individualized treatment plans.
• Facilitate effective communication among various stakeholders, including supervisors, team members, family members, and community partners, to discuss patient progress and adaptations needed for treatment.
• Develop and maintain a tentative scheduling system to arrange appointments, home visits, and observations, ensuring optimal follow-up and monitoring of client progress.
• Coordinate and execute home visits, in-home observations, staff interviews, and data collection to provide comprehensive care and insights into the patient’s environment.
• Oversee the implementation and effectiveness of treatment plans, adjusting strategies as necessary to meet the evolving needs of patients.
• Participate in Interdisciplinary Team Meetings and related discussions to align care strategies and share insights across disciplines.
• Prepare detailed monthly and quarterly progress notes to track patient development and inform future treatment directions.
• Organize and conduct training sessions to ensure staff members are proficient in program plan implementation and documentation.
• Provide ongoing support through follow-up interviews and home visits, fostering continued engagement and monitoring of the patient’s mental health journey.
• Act as a crucial liaison with the public and the larger mental health community, working towards enhancing treatment strategies and developmental goals across the organization.

As an LPC with Kid Serenity Health Care, you will engage with diverse client populations, including both adults and youth, allowing you to make a meaningful impact across a wide range of mental health concerns. This role is essential in addressing various issues, including trauma, substance misuse, and relational challenges, ultimately guiding clients toward healthier, more fulfilling lives.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Possession of active and unrestricted Professional Counselor (LPC) licensure in Georgia.
• A minimum of two years of direct counseling experience, ideally within a clinical context.
• Strong proficiency in family therapy, couples therapy, trauma-informed care, or substance use treatment is advantageous.
• In-depth understanding of legal and ethical standards related to mental health care.
• Exceptional interpersonal and communication skills, with the ability to build rapport with clients from diverse backgrounds.
• A proven track record of collaboration within a healthcare team setting.
• Familiarity with electronic medical record (EMR) systems and related technologies.

Experience in community health settings or a similar environment, showcasing expertise in delivering culturally competent care, will be highly valued. Specialization in specific therapeutic modalities or populations can provide additional context to your counseling practice.
