Job description

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care and to minister to the unique needs of each individual with respect, dignity, and compassion. Full-time: Neville Island & surrounding areas
The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of a team to give direct nursing care to patients requiring intermittent visits. The LPN collaborates patient care with the case managing RN, physician, and patient and follows a patient specific treatment plan.
QUALIFICATIONS
  • Graduate of state-approved school with course in pharmacology
  • Current license in the State of Pennsylvania
  • Minimum of two [2] years experience in patient care
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the state

DUTIES AND RESPONSIBILITIES
Clinical Skills
  1. Provides skilled nursing care that includes activities such as teaching and treatments (including IV therapy- if IV certified, venipuncture and complex wound care) and health promotion practice
  1. Executes preventive, rehabilitation and restorative nursing procedures appropriate for the patient's care and safety
  1. Assesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, changes in patient's physical, mental or emotional condition to the physician and the supervising Registered Nurse
  1. Evaluates the patient and home situation to determine the health teaching required at each skilled visit.
  1. Instructs and educates the patient and the designated representative regarding the care requirements of the patient in the home. Conducts a comprehensive physical examination and nursing assessment of the patient at each skilled visit.

Documentation
  1. Tablet is synced within 24 hours.
  1. Updates the medications listed in the electronic health record on an ongoing basis
  1. Addresses all areas of the skilled nurses note at each visit, including a comprehensive pain assessment as needed
  1. Documents all verbal orders obtained from the physician as they are received
  1. Completes a hospital hold coordination note as soon as he/she is aware of the transfer
  2. Documents all communications with the physician and the interdisciplinary team on the patient's electronic health record.
